Northwich Primary Care Network (PCN)

Northwich Primary Care Network (PCN) is the name given to the organisation of GP practices who are working together to provide better, more co-ordinated, effective and sustainable care for the people who live in Northwich.  

By working together and sharing resources we are employing more skilled staff than ever before so that we give people the right care at the right time. 

There are seven practices in our Primary Care Network (PCN):

Oakwood Medical Centre

Together, we are responsible for the care of over 70,000 people in Northwich and the surrounding areas, working to create a better health future. The PCN consists of all the teams who work in these surgeries; we also have some staff who work across the whole of the PCN.

Home Visits

If you are housebound or elderly and too ill to come to the surgery, a health professional will visit you at home. Telephone 01606544342 before 11am, if possible and give the receptionist brief details of the illness so that the doctors can plan their visits according to the degree of urgency. Cervical Smears

A computerised system is run by Cheshire Family Health and you will automatically be advised by post when your smear is due. You will be asked to make an appointment with one of the nursing team for your cervical smear.

Self-Refer Services

Social Prescribers
Part of your GP Practice team and can provide non-clinical support to help you improve your health and wellbeing.

They will work with you to identify what matters to you and connect you to services, community groups and other local resources.

Social Prescribers can help with:

-Finding local groups and making friends
-Accessing work, training or volunteering
-Money or housing difficulties
-Healthy lifestyle choices
-Support for carers
-Accessing mental health support

Vasectomy

A vasectomy is a safe, effective and permanent solution to contraception . The procedure takes about 15 minutes.

Self-refer by calling the Cheshire West and Chester Integrated Sexual Health Team on 03002470020.

Physio

Pharmacy First

This service can be provided to patients who present as a walk-in or referred by St Johns. 95 percent of pharmacy’s  offer this service. Medication is free for those who are exempt from prescription charges.

Minor Eye Service
The Primary Eyecare service will see urgent and minor eye problems under the NHS.

Non NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Please note: Passports and shotgun licences are no longer signed by the doctor.
